Throughout the year, the Company offers free WellFairs — health fairs featuring on-site screening tests to all employees, retirees, and spouses.

The Company's WellFairs can include the following screening tests:

• Height,Weight, Body Mass Index (BMI)
• Blood Pressure
• Body Fat Screening
• Blood Testing (slight variations in specific tests may vary at some sites depending upon vendor capability)

All results are strictly confidential between you and the health fair vendor. The results of your screening will be either provided at the health fair or mailed to your home from the vendor.

Choose-to-Bring Incentive

As the 2009 WellFairs kick off in January, a new incentive program — Choose-to-Bring — is designed to help us attain 100% participation. Employees who bring or refer an employee to a WellFair who has never attended one in the past will receive $100. In addition, the new attendee will receive $100. WellFair attendees can then earn an additional $50 by completing an online health risk assessment.

This new incentive complements our wellness program's strong slate of benefits — fitness and weight-loss incentives, smoking-cessation program, free flu shots and mammograms — and other resources available to help you attain your health goals.

Health Risk Assessment

Below are the steps to access the HRA for all employees, retirees, and spouses who are enrolled in Anthem's EPO and PPO plans.

1. Go to www.anthem.com.
2. Click on "Member" tab.
3. On "Member Welcome" page, click on the "Register" button.
4. Complete registration.
5. Click on "My Health" tab.
6. Click on "Health Assessment."
7. Click on "Take Assessment" to begin the HRA.

Below is the process for employees, retirees, and spouses who have waived medical benefits through Anthem.

1. Go to www.webmdhealth.com/sponsors/eon.htm.
2. Complete registration
3. Click on "My Assessment" tab
4. Take assessment
