E.ON U.S. - Wellness - ChooseWell Grant Application

Choose Well. Be Well...in 2008!

E.ON U.S.'s ChooseWell Grant supports employee involvement in community and workplace wellness initiatives targeting weight loss, nutrition, fitness and tobacco cessation. Managers may apply for up to $1,000 to fund such initiatives.

ChooseWell Grant Criteria:

1) The activity must target the ChooseWell goals of lowering BMI and glucose levels and support the 2008 Choose2Act Incentive.

2) Wellness initiatives must be open and available to all employees reporting to the requesting manager.

3) Results must be documented and shared (in the aggregate) with Corporate Health & Safety.
