Since the launch of E.ON U.S.'s Wellness program in 2004, the company has offered a variety of programs to support the health and well-being of you and your families. Not only are you improving your health through these programs, you are also enjoying the associated activities that bring you together with your family and colleagues.

A ChooseWell incentive will be awarded to full-time employees and regular part-time employees for:

  1. attending a company health fair in the first half of 2009, and
  2. completing an Anthem online health risk appraisal (HRA) by July 31, 2009.
The ChooseWell incentive will be in the amount of $50 payable on your paycheck in December 2009.

To receive your 2009 ChooseWell incentive, follow these steps:
  1. Attend a WellFair.
  2. Complete an online Health Risk Assessment through one of the following sites:


  3. Receive $50 on your December 2009 paycheck. *
* Please note that only regular, full-time and part-time employees as of Dec. 1, 2009 are eligible for the incentive offer. Spouses and other family members are not eligible. According to IRS regulations, the incentive is considered taxable income.

How to Complete a Health Risk Assessment

Below are the steps to access the HRA for all employees, retirees, and spouses who are enrolled in Anthem's EPO and PPO plans.
  1. Go to www.anthem.com.
  2. Click on "Member" tab.
  3. On "Member Welcome" page, click on the "Register" button.
  4. Complete registration.
  5. Click on "My Health" tab.
  6. Click on "Health Assessment."
  7. Click on "Take Assessment" to begin the HRA.
Below is the process for employees, retirees, and spouses who have waived medical benefits through Anthem.
  1. Go to www.webmdhealth.com/sponsors/eon.htm.
  2. Complete registration
  3. Click on "My Assessment" tab
  4. Take assessment
